Weeki Wachee River Giant SUP Experience (Motorized)

RENTER WAIVER & ACKNOWLEDGEMENT FORM   1.    I wish to rent this motorized giant stand up paddle board (here forth referred to as “boat”) being operated by The Company under the name "Weeki Wachee River Giant SUP Experience", and I acknowledge that this rental is completely voluntary.    2.    I UNDERSTAND THAT THERE ARE INHERENT RISKS INVOLVED WITH BOATING, including, but not limited to equipment failure, perils of the sea, harm caused by other vessels, acts of fellow participants, entering and exiting the water, boarding or disembarking boats, and activities launching boat.    3.    I fully assume responsibility for my own safety, the boats’ safety, the return of equipment (see below) and verify that I am physically able to rent this boat. Equipment Return Checklist: □   Giant Stand Up Paddle Board □   Motor Remote □   3 electric trolling motors + batteries □   two bean bag chairs □   Artic cooler □   2 paddles □   2 life vests   4.    I acknowledge that failure to return part or all of the rented items, or returning items damaged, may result in a replacement fee.   5.    I fully assume liability for safe boating practices and following provided tips below.     Tips: □   Stay away from shallow and grassy water □   Keep remote around your neck to avoid losing it □   Be ready to stop motor at all times in case of emergency □   Make left at end of canal to go against current using motors.  This way of motors dies upon your return you are carried by current with exception of canal.   6.    In the event of distress, I will not hold any person or entity responsible for their actions in attempting to perform rescue.  I acknowledge there may be a fee of $100/hour for rescue missions.   7.    Having read this wavier, I agree to RELEASE from LIABILITY and HOLD HARMLESS The Company, along with its respective officers, directors, employees, owners, members, managers, affiliates, agents, representatives, attorneys, heirs, personal representatives, successors and assigns, all individuals associated with the Boat Ride, and all individuals and entities having an interest in the boat being used for the Boat Ride, from any and all liability, claims, demands, equitable relief, damages, costs, expenses, and causes of action of any kind or character, of any type or nature whatsoever (including negligence), arising out of the Boat Ride.  8.    I authorize The Company to use any photographs, audio and video recording related to my boat rental for any and all purposes.   9.     I certify that I have read this document and agree with its contents.
